How do I book a course?
To book a MiDAS course you firstly need to complete the medical process, please complete the following form:
Then return to the Occupational Health Unit (OHU) in an envelope marked ‘Confidential’ to the address located on the form. Once you have been declared fit to drive by OHU you will be advised to complete the registration form (see below) and return it to the Road Safety Team Admin Team who will then contact you, or the organisation who has arranged this for you, to arrange a suitable date for the training.
Then return them to the Road Safety Team Admin Team in an envelope marked ‘Confidential’ to the address located on the form and at the bottom of this page. We will then contact you, or the organisation who has arranged this for you, to arrange a suitable date for the training.
If you need to obtain the D1 entitlement on your licence, the MiDAS administrator can provide details of a local assessor who can help you with this.
If you do not have the D1 and have no intention of gaining this entitlement, you can do the MiDAS training for an MPV certificate (Vehicle with up to 8 passenger seats). This can be done with the wheelchair accessible module too, so even if you cannot drive a minibus, you will be able to operate the tail lift etc.
If unsure as to which course you require contact the MiDAS admin team on the telephone number below.
